We are excited to announce a time of growth and positive change for our communities as we unify and increase access to Primary Care.
In June 2021, St. Vincent Medical Clinic and Rocky Mountain Family Practice (RMFP) merged operations under a new practice named St. Vincent Family Health Center. The Family Health Center is owned and operated by St. Vincent Health (SVH) and offers primary care and urgent care.
The high level of service you are accustomed to receiving will continue and expand with this move into the new Family Health Center. The care, providers and staff you have come to know are available to you at St. Vincent Family Health Center.

Care and services include:
- Primary care for all ages, from infancy to seniors
- Women’s health, including pregnancy, prenatal care and maternity care
- Men’s health
- Pediatric care
- Employment, DOT, and sports physicals
- Vaccinations for children
- Diabetes management
- Flu shots
- Veteran’s Choice Plus services
- Workers Compensation
- Altitude sickness care and oxygen therapy
- Chiropractic care
- Mental health services
- Sports medicine
The St. Vincent Family Health Center practice philosophy is to set the standard for evidence-based and patient-centered family practice care in Lake County and the region. In addition to seven-day-per-week primary care and walk-in urgent care, the practice also offers telehealth and house calls.
